The Patient Portal is a way for you to communicate online with your community health center.
Registered patients will have secure and confidential access to the website and may:
- Request an appointment
- Request a medication refill
- Ask your medical team a question
- Receive test results and information online

To get started with the Patient Portal, visit your health center and ask for an account to be created.
- All you need is your email address. We will create a NextGen account for you and give you a personal enrollment token (an 8-digit number used to sign in).
- Once we create your account, you will receive an email with easy instructions on how to enroll. That's all there is to it!
